Establish Your Preferences for Health Care

You should always have a choice when it comes to important medical decisions. As you get older or your health declines, you may be put in situations where you are not be able to speak or express your wishes. Fortunately, you can still make sure you have control over how your care proceeds through advance directives.

Advance directives are legally binding documents that outline your wishes for medical care. You can use advance directives to instruct doctors on how to proceed if you are unable to communicate, or you can name a trusted friend or family member to make those decisions on your behalf. You do not need an attorney to create advance directives.

We offer free hour long appointments with trained facilitators if you’d like assistance in completing the document. If you’d like to make an appointment, please call 800- 828-3627 and ask for an appointment with an Honoring Choices facilitator to complete an Advance Directive.
