Thank you for choosing St. Croix Regional Medical Center.  If you would like to make an appointment, you may call us directly at 715-483-3221 or 800-828-3627.  This convenient, online appointment form allows you to request non-urgent appointments at SCRMC.

Simply fill out the electronic form below and submit it to our schedulers.  Your request will be processed as quickly as possible between 7:30 AM and 4:00 PM, Monday –Friday.

The time and day of your appointment will be confirmed by phone or email. Please note that to contact you by email requires an authorization signed at one of our clinics.

To cancel or reschedule your appointment, please call the clinic number where you made your appointment or 715-483-3221 or 800-828-3627. The appointment is scheduled after you are contacted by phone or email to confirm the time and day.
