If you have been diagnosed with diabetes, you know that staying healthy with this disease can have special challenges. SCRMC now offers a Diabetes Care Clinic in St. Croix Falls using a “TEAM” approach for achieving clinical excellence in diabetes care. During appointments, patients meet with a selected team of providers. That can include a diabetes clinic nurse, a diabetes educator, and a dietitian,as well as other providers who may be needed such as sports medicine, podiatry, physical therapy, and a social worker as needed. Certified Diabetic Educators Robin Herr, Dietician; and Kathy Johnson, RN plus staff member Deb Voss, RN are members of the team.
Care includes measurement of important quality indicators such as: A1C and LDL levels, blood pressure, and foot care. A pharmacist, Anita Lundquist, will also meet with patients to review their medications for effectiveness, and to address cost barriers. Dr. Marsha Beyer is the program's leader working with Dr. James Wallace, Internal Medicine and Jean Roedl, ADM-BC and Nurse Practitioner.
Diabetes Care Clinic staff see patients Tuesday afternoons at the St. Croix Falls Clinic. To schedule an appointment or request information, call 800-828-3627.
